New Patient Info
David N Gavin, DPM, PA is accepting new patients.
Insurance verification is recommended prior to your appointment. This can be done by contacting your insurance company (the phone number is on the back of your insurance card). Payment of all Co-Pays and Deductibles is the patients responsibility and will be collected the day of services rendered.
We ask that all new patients arrive a minimum of 20 minutes prior to their scheduled appointment in order to complete the registration process. Patients who have already completed the registration forms (by clicking on the links here on this page or by picking them up at our office) need only arrive 10 minutes prior to their scheduled appointment time.
Scheduling and Cancellations: Please call our office or click here to schedule an appointment. If you are unable to make your appointment, please notify us 24 hours in advance.

- List of all current medications

Medicare as Primary insurance (with ALL Medicare secondary insurances)
Additional Insurance Information
Medicare covers almost all of your foot care health needs, however please be aware there are a few procedures that Medicare will not cover.
If Medicare does not cover the procedure your supplemental insurance will not cover either.
Please see the exclusions below:
Routine Foot Care: Is not covered; unless there are medical code class findings which the Doctor will discuss with you.
- Routine foot care includes; trimming or removing corns or calluses, trimming, cutting, clipping or debriding of nails
Custom Orthotics: Are not covered, corrective action for flat feet
